Y can be whatever...
At the moment it seems that Y is 0,01 (2.0 + 0,01 = 2.01) but can att a later stage be something elese.

My question is if updates (update R2 versions) must be installed manually (how it's done*) or if there is some "install.exe" doing it?

(*)Or just update files in same folders or if it may differe from time to time?

Yax.... this became only stupid. (erased former answer)

...but why not exclude all "settings files" (and exclude 3rd party modules that has no updates) from the new version and just overwrite it all?

minor version upgrades simply require replacing the php files. only major releases require running the upgrader - which is built into the installer.
